[Date] complete this form daily during the permitted period; and (5) Current certifications of the Operator from Wake County and PHTA or an equivalent course as determined by the Director
[Date] immediately upon the termination of the Operator. The pool shall be closed until an Operator is employed or contracted. An Operator of a pool shall register annually with the Department and list all pools under the Operators' responsibility
