Running Head: INFLUENCES ON MY NURSING CAREER C304 INFLUENCES ON MY NURSING CAREER C304 Task 1 Western Governors University A. Nursing Theory The nursing theory that has most influenced my values and goals would be Florence Nightingale’s theory of environmental adaptation. A1. Excellent Nursing Practice I believe most nurses try to emulate the practices and theories of Florence Nightingale because they are still prevalent and used in today’s practices. Nurses today utilize the “canons of nursing that guide the practice of professional nursing” (Cherry & Jacob, 2019, p.81). Of Nightingale’s canons or standards, nurses today implement most of them. The important ones that are used for patient care are noise, variety, diet, light, chattering hopes and advices, and cleanliness. As nurses we apply these canons in a variety of ways. Noise, as Nightingale explained is “the unnecessary noise, or noise that creates an expectation in the mind is that which hurts a patient” (Cherry & Jacob, 2019, p.81). Many times, while we are attending to multiple patients, sometimes we are not aware of how much noise we project. We must be vigilant in our practice to use noise appropriately so that patients are in a quiet environment to facilitate healing. Variety is another way that nurses today implement better patient care. Most hospitals endorse pet therapy programs that promote healing and “alleviate the boredom felt by those suffering (Cherry & Jacob, 2019, p.81). Nurses have also been creative in their approaches to INFLUENCES ON MY NURSING CAREER patients that seem depressed by filling their rooms with words of encouragement that are colorful and bright with the hopes it distracts the patient from their situation. Nurses today have also been vigilant about promoting healthy diets in conjunction with registered dieticians to provide specific diets to promote healing. Nightingale was an advocate for finding ways to access good, healthy food which would in turn help reduce recovery times. I think one of the most important canons Nightingale implemented is that of light. She expressed that “it is the unqualified result of my experience with the sick that second only to their need of fresh air is their need of light” (Cherry & Jacob, 2019, p.81). Nurses continually offer sunshine therapy when the patients are stable. As a nurse, I try to always have the blinds up and position the patient so that they may be able to see outside. Chattering hopes and advices instills in nurses to not give patients false hopes or “[make] light of their danger and by exaggerating their probabilities of recovery” (Cherry & Jacob, 2019, p.81). By promising patients that they will get better and they don’t, the nurse has been untruthful and not practicing within the nursing code of ethics. The last canon of Nightingale is cleanliness. My perception of this canon is that it was the most important factor in patients recovering better and faster. It’s amazing as a nurse to see the fulfilment of a basic need for a patient such as a shower, it changes the patient’s perspective and their “feeling of well-being after washing and drying” (Cherry & Jacob, 2019, p.81). Nightingale also emphasized the importance of washing hands, this simple habit, now standard practice has greatly decreased infection. A2. Professional Nursing Practice Theory Nightingale’s definition of nursing is that “nursing is an art-an art requiring an organized, practical, and scientific training … putting us in the best possible conditions for nature to 2 INFLUENCES ON MY NURSING CAREER The ANA is responsible for enforcing high standards at the forefront of nursing practice, advocates that nurses have a safe and ethical work environment, supports the health and wellness of nurses, and supports issues with health care that affect both the public and nurses (“About ANA,” 2020). The state board is responsible for addressing concerns about a nurse’s practice as well as awarding, renewing, and taking disciplinary action when necessary on nursing licenses (Cherry & Jacob, 2019, p. 68). C2. Influences of the State Board of Nursing and ANA The ANA influences my nursing practice by giving me the tools needed to advocate for my patients, the security of being in a safe work environment, and directly impacts my goals and values with patient care. The State Board of Nursing influences my nursing practice by keeping me responsible and accountable for my actions as a Registered Nurse. If I were to violate these rules, my license could potentially be suspended or terminated. C3. Requirements for License Renewal in State I currently hold a license in the state of Utah. To renew a license in Utah, one must have been awarded a Registered Nurse license in Utah, complete an online suicide prevention training, and requirements for continuing education must be met. The three following options to obtain continuing education credits are: thirty hours for every license period, four hundred hours of licensed practice, or two hundred hours of licensed practice and fifteen contact hours (“Utah Department of Commerce Division of Occupational and Professional Licensing”, 2020). C3a. Consequences of Failure to Maintain License Requirements in State Failure to maintain the Registered Nurse license depends on the time the license has been expired. The license is valid for two years and if the applicant has not been in practice for two to five years, they must have documentation of current compliance. If it has been more than five 5 INFLUENCES ON MY NURSING CAREER years since the applicant has practiced, they must take the NCLEX-RN and pass within sixty days or they can enter a nursing a program. If it has been longer than eight years since the applicant has practiced, they must enter a nursing program and take the NCLEX-RN and pass (“Utah Department of Commerce Division of Occupational and Professional Licensing”, 2020). C4. Difference in Requirements for Compact and Non-Compact State Utah is part of the Nurse Licensure Compact (NLC). Having a license in a compact state makes it possible for a nurse to practice in any state that is also part of the NLC without having to obtain additional state licensure (Cherry & Jacobs, 2019, p. 67). If a nurse wants to practice in a non-compact, they would have to obtain a license to practice in that state. D. Differences Between Food and Drug Administration and the Center for Medicare and Medicaid Services The Food and Drug Administration (FDA) was established to ensure safety and security for not only food products but the use of medical products as well whereas the Center for Medicare and Medicaid Services (CMS) oversees state and federal health insurance programs. D1. How the Two Agencies Influence Professional Nursing Practice The FDA influences my nursing practice by making sure the medications I am giving my patients are safe and provide benefits without harmful side effects and that they are aware of them. The CMS makes it possible to provide care to those who have limited finances in addition, it ensures my patients have safe and reliable medical equipment and services (Cherry & Jacobs, 2019, p.103). D1a. Role as Patient Advocate in Promoting Safety in Alternative Therapy 6 INFLUENCES ON MY NURSING CAREER Alternative therapies are available to patients and can be used at their discretion. Being a patient advocate, it is important to advise and educate the pros and cons of such therapies. Although the decision is up to the patient, it is important for me to be supportive of what they decide. E. Purpose of Nurse Practice Act in State and Impact on Professional Practice Nurse Practice Acts (NPA) are guidelines which include the Registered Nurse scope of practice, education, and the grounds for disciplinary action for those nurses that violate any portion of the nurse practice act (Cherry & Jacob, 2019, p. 66). The NPA has influenced my nursing practice by having set standards that provide a safe and ethical work environment and allows me to know what I can and cannot do regarding patient care. E1. Scope of Practice in State The scope of practice outlines procedures, processes, and actions within the professional license that allows the RN to legally perform (Cherry, & Jacob, p. 66). The Utah scope of practice includes medication administration, treatment, consultation, assessment of health status, and maintaining effective and safe patient care to name a few (“Nurse Practice Act,” 2013). E2. Define State Delegation for RN The state of Utah defines delegation for nursing practice as it falls under the licensed nurse’s scope of practice. The nurse is able to delegate certain tasks to either a Licensed Practical Nurse (LPN) or a Certified Nursing Assistant (CNA). For example, the RN can delegate to the LPN to give an oral medication and can delegate obtaining vital signs to the CNA (“Nurse Practice Act,” 2013). 7 INFLUENCES ON MY NURSING CAREER team. Empathy is an important component within a team because it ensures that others feel heard and understood. Having self-awareness on a team allows better interaction between staff and ultimately better decision making. H2. How Work Environment Impacts the Following I have excellent nursing leadership in my current position. My work environment facilitates personal and professional growth which makes me feel fulfilled and satisfied. I feel that I am a valued member of my team which makes everything run smoothly and that I can give more effort and energy to problem solving and continued leadership. My team includes a multitude of specialties from pharmacy, doctors, to staff members and managers. If there is a decision that needs to be made, I can reach out to any of these team members to help problem solve, give support, and make critical decisions when needed. When there is a positive work environment critical thinking and decision making are more effective. Even though I have been a Registered Nurse for over two years, I am constantly learning new things. My work environment is vast and influences me to challenge myself, gives me educational opportunities, and the opportunity to learn new skills. In addition, my work environment encourages personal and professional growth and allows me to feel that I am competent in every aspect of my position. References About ANA/ ANA Enterprise. (2020).
