You are asked to see Mr. Sean Cathal Laurence Carter, a 63 year old smoker of 50 pack years
with a 3 day history of increasing confusion and drowsiness. He has been taking no
medications. Clinically he appears unwell and cannot provide a history. There are several
large lymph nodes in the right supraclavicular fossa and 5 fingers of hepatic enlargement but
no other clinical signs. His biochemistry (in mmols/l) shows :

(a) What abnormality does his chemistry show?
(b) What immediate treatment will you institute?
(c) What other urgent investigations will you perform?
(d) What unifying pathology can explain the presentation?
2. A 26 year old professional woman develops a red scaly rash on her elbows, knees, forearms
and scalp. This gradually extends over a number of months to affect her face, lower back and
lower limbs. It is very unsightly but not very itchy. She also has some pain and stiffness in her
hands and knees. She previously experienced a red scaly rash following a sore throat as a
teenager but this resolved spontaneously within a few weeks.
Her father has a history of a similar rash on his scalp.
(a) What is the likely diagnosis of the skin disease and why?
(b) This skin disease can present in different forms. Describe these clinical variants
(c) Describe the treatment options available to this woman and the advantages /
disadvantages of each option.
(d) Discuss the overall impact this skin disease may have on this woman’s quality of life.

  2. A 55 year old carpenter complains of a six month history of progressive difficulty using his right hand, such as manipulating nails. He denies any tremor, sensory symptoms or pain A. Based on the above information only, list 3 possible differential diagnoses. Discuss your reasons in each case. In your subsequent neurological examination, you note wasting and weakness of the right hand, with brisk reflexes, but normal sensation. B. Based on this additional information, discuss a. Your revised differential diagnosis, highlighting the most likely diagnosis b. Relevant investigations

  3. A 25 year old woman presents with weight loss and diffuse shotty lymphadenopathy. Her ex partner was an injection drug user. Your suspicion that she is HIV-infected is confirmed with a positive HIV test. On follow-up testing her CD4 count is determined to be 150 cells/mm3 and her HIV viral load is 120,000 copies /ml. She is otherwise well with no other abnormal findings on examination. Please answer all of the following questions: (a) What potential treatment would you discuss with her and how would you proceed with initiating these (b) What routine additional health screening including blood tests would you obtain. Are there any vaccinations that you would consider? (c) She is very concerned about this diagnosis and asks you specifically about her prognosis. How would you counsel her? (d) She is in a new relationship and is very anxious to have children. What information would you give to enable her to make informed decisions regarding her ability to safely become a mother?
