I wish to make application to participate in the Aloha Driving Schools Traffic Safety Program. I am willing to give the necessary time
and effort in order to fulfill the requirements of this course as printed in the Aloha Driving Schools Philosophy on the reverse side of
this application.
It is further understood that it is necessary to pay $490.00, refundable only if I withdraw, in writing, up to the first day of class, minus
a $25.00 processing fee. The full $490.00 sum is due at time of enrollment, NOT by the first day of class. Please make checks payable
to “Aloha Driving Schools”. Refunds will NOT be granted for any reason for “dropping” the program after the first day of class, for
suspension, for expulsion, for failing the class, or for not completing the course within the dedicated timeframe.
I understand that I am required to drive an additional 50 hours outside of class with a parent/guardian in order to get my license.
I understand that I must have and bring my Washington State Instruction Permit to every behind-the-wheel (BTW) lesson. To cancel
a pre-arranged BTW lesson, I must notify the office at least one day in advance. If I do not bring my driving permit, do not bring my
required glasses/contacts, do not appear for a drive, or do not notify the office within the 24 hours before the drive time, I will pay
a $30.00 no show fee before the next drive. Students needing additional drives to pass the required skills will need to pay $45.00
per hour for each additional BTW lesson.
I have read and agree to comply with all of the philosophies, policies, and requirements as printed on the reverse side of this
application, including the “Intermediate License Restrictions”. If I have questions or concerns, I must contact the office or my
instructor within the first week of class.

As the parent/guardian, I fully approve of my student enrolling in the Aloha Driving Schools Traffic Safety Program and I will provide

Aloha Driving Schools’ driving instructors. An average of 2 to 3 hours of adult -supervised driving to each one half hour behind-the-
wheel lesson is recommended.

Aloha Driving Schools Policies – WAC 308- 108 - 120 Aloha Driving Schools Philosophy

1. Driving is an important, lifelong skill. The student needs to be serious about this course and make it a high priority. This means that they may need to free up their schedule in order to be successful. 2. We encourage parents to become involved in their student’s educational progress. You are paying for this service, so please be involved and make sure your child is getting the full benefit of this service. 3. Please feel free to call the office or instructor about your child’s progress during this course. 4. If you have any comments or concerns, please give us a call or feel free to talk with us in person. Enrollment Policies 1. Students must be 15 years or older by the first day of class. We refuse to allow a student to attend our traffic safety program before they turn 15 years old. 2. Course duration is a minimum of 5 weeks and a maximum of 18 weeks, consisting of 15 periods of 2 - hour classroom instruction sessions, 6 one hour long behind-the-wheel drives, and one hour of observing another student drive. The total cost of this course is $ 4 90.00. 3. Students shall not be enrolled into a driver’s education course after the third classroom session of that course has been completed. 4. Students will receive no more than 1 class period of instruction per day and no more than 2 hours of makeup time per day. 5. The student will be notified by phone in the event the school cancels classes. The student may also be texted, emailed, or see a notification on our website about class being canceled. Canceled classes will be made up at the end of the course, or during an alternate day. 6. Classes are usually held from either 3:30 p.m.-5:30 p.m. or 6 p.m.- 8 p.m. during the school year, although class times may differ from these times. 7. A refund of all fees paid will be made, upon request, if school is not capable or willing to begin instruction within 30 calendar days from the agreed to in the contract. 8. Students who miss more than 3 class periods WILL be un-enrolled and will need to restart the course completely, joining a different class and paying another $ 4 90 for the course. If the student was absent for their 4th^ and onward missed days due to illness, injury, family emergencies, etc., please talk with the instructor or office staff. 9. Aloha Driving Schools does not guarantee that any student will pass the Washington State license examination, or that the student can obtain a license.

  1. A fee of $45 will be charged for any insufficient funds or stopped check that is returned by a bank. The issuer of the check will be responsible for legal expenses. 11. Students are fully accountable for their conduct. Any damage done by a student to school property, building, or parking lot will result in the student being immediately expelled from the program with no refund. Restitution for any damage will be the responsibility of the student, parent, and/or guardian. 12. Cell phone usage or sleeping is not allowed during classroom sessions or in the vehicles during drive or observation time. Anyone found talking/texting on a cell phone or sleeping during these times will be considered absent and will not be given credit for their attendance. All students hereby agree to keep the building, parking lot, and automobiles clean from all litter and debris. 13. Any students found with reasonable suspicion of being under the influence of alcohol or drugs while attending classroom or behind-the-wheel instruction will be immediately expelled from the program with not refund. A conference will be held with their parent/guardian and local law enforcement officers will be contacted. 14. All students are expected to be responsible and self-governing. Attitude maturity is an important part of this course and of being a responsible driver. Any student who was found purposely disruptive or disrespectful to the instructor, other students, or with anyone on Aloha Driving Schools’ property shall be dismissed from and counted absent from the session they are attending, or expelled from the class depending on the severity of the action. Any students found with reasonable suspicion of gang activities, fighting with other students or staff, bringing weapons to class or in the instruction vehicle will be expelled and reported to law enforcement and no refund will be given. 15. Smoking is not allowed on school property. Refund Policy For notice of withdrawal received by the first day of the traffic safety course, a full refund will be issued within 3 business days minus a $25 processing fee. The request must be made in writing to obtain a refund. No refund of tuition will be granted for the traffic safety course after the first day of class.

“Aloha Driving Schools” Policies Regarding Examinations

1. If you have any physical, mental, visual, or medical issues, please let the office staff know. You may have to be tested at the Department of Licensing. Failure to notify staff may result in the invalidation of your exam scores. If you received a letter from the DOL to take an exam due to physical, mental, visual, or medical issues, you must test at a DOL office. Failure to disclose this information prior to testing may result in your test(s) not being honored at the DOL and you will **not be refunded the fee(s).

  1. At the time of the test, the applicant must provide proof of identification such as a photo permit, ID, license, school ASB** card, or birth certificate and social security card. The applicant must also provide their confirmation ID/permit/license **number at the time of the test.
  2. We reserve the right to refuse service to anyone.**
  3. The fee for the Knowledge exam is $30.00 per attempt on Saturdays from 12-3 pm (we won't start tests any later than 2: pm) or during the week (by appointment) $50 per attempt.
  4. The fee for the Skills exam is $ 50 .00 per attempt.
  5. The fee for using the school’s training vehicle for the exam is an additional $50.00 for each attempt when the school’s training vehicle is used. *These fees are separate from the fee charged for your Traffic Safety Course and DOL’s applicant and licensing fees
  6. Your exam scores will be entered into the DOL database within 24 hours. You will have to visit the DOL office to apply for your license. Passing the Knowledge and Skills exams does not guarantee that an applicant will be issued a driver’s license. The Department of Licensing will determine if an applicant has met all licensing requirements. If you are under the age of 18, you must be at least 16 years old, successfully completed a traffic safety education course, have had your permit for at least 6 months, have had 50 hours of driving experience with at least 10 night driving hours, and have passed both your written and skills test to apply for your driver’s license.
  7. Refunds will be issued upon request before the applicant has begun testing. We do not issue refunds after testing has begun.
  8. The Department of Licensing reserves the right to conduct random re-examinations; applicants refusing to take a re- examination may have their license revoked.
  9. If you have any grievances/complaints, submit them in writing and the owner will review them within 7 business days. If you are unable to submit a complaint to the office in person, please email us at [email protected] or call/text us at 360- 633 - 3636.
  10. If an applicant fails either test, they may retest as soon as they’d like, granted that an instructor is available.
  11. The Knowledge exams consists of 40 multiple-choice questions about traffic safety laws. Applicants must answer at least 32 questions correctly to pass the exam.
  12. The Skills exam will test the applicants’ ability to drive legally and safely. Applicants must obtain 80% or higher in order to pass the Skills exam.
  13. For the Skills exam, your car must be legal to operate on public streets. This includes, but is not limited to: windshield, seat belts, windshield wipers, defroster, tires, mirrors, all brake and reverse lights, turn signals, current and valid insurance, and be legally licensed/tabs current.
  14. You will need to demonstrate proper arm signals prior to beginning the Skills exam.
  15. Pursuant to RCW 46.20.720 a driver convicted of an alcohol offense may be required to have an Ignition Interlock Device (IID) in order to drive. If the applicant has an IID in their vehicle, he or she must take their skills exam in a vehicle with an IID and must use the IID during the exam.
  16. Applicants wishing to use their own vehicle for their skills exam are required to provide proof of insurance and current registration, (current tabs), on the day of the exam.
