"X. Wake Forest" Baptist Health Bacillus Calmette-Guerin vaccine (BCG) Immunotherapy Protocol BCG is used for treatment and prevention of carcinoma in situ of the urinary bladder and prevention of Primary or recurrent slage. BCG treatmentis an absorplion mechanism of delivery within the bladder wall. BCG is a tuberculosis vaccine containing active tuberculosis and requires strict compliance with Full PersonalProtective Equipment (PPE) Urine sample for urinalysis must be evaluatedby a provider prior to administration. - Provider may reschedule or modify the treatmentif there are any signs and far symptoms of infection or gross hematuria * Urine to be sent for culture,and reschedule for follawing week. Dosing- $0 mg of BCG per treatment Initially-6 weekly treatments Repeat Cystoscopy after 6 treatments Maintenance therapy-3 weekly treatments Repeat Cystoscopy 6 weeks after 3 weekly treatments Treatments alter 3 weekly inslillations TBD by MD Procedure Obtain patient labelfor documentation and charges Gather equipment for mixing medication PPE requirements include: ° Gloves nonsterile * Face mask wilh shield = Waterproof gown Don PPE and mix medication under Sterile Guard Chemo Hood Equipment for mixing the medication + 3cesyringe * 60 cce syringe * SOccnormalsaline vial - #18 gauge needle + Air valve filter comes with medication) + Biohazard bag - Alcoholswabs - Adapter tip Place all above equipment in the hood Remove medical’nJn from refrigerator, count and document number remaining. Draw up 3 cc of normalsaline and pul into BCG vial-mix Oraw up and inject back inlo the SOcc vial of normal saline Use an 18 gauge needle to draw BCG mixture up into a 60cc syringe Once the BCG mixture is draw up remove 18 gauge needles with a blue adapter tip Place BCG mixture in biohazard bag Insert a#12 Coude red rubber catheter to instill BCG mixture Post procedure Instructions: Instruct patient to hold in bladder for at least 2 hours if possible Urinale in commode at home add 2 cups of Clorox bleach in toilet Let it sit for 20 minutes and then flush. Repeat this process for the next two times the patient urinates.(Total=3)
