Azathioprine (Imuran) Right Reason Right Reason Azathioprine is an immunosuppressant drug used off the label to treat Crohn's disease with the aim of improving a patient's quality of life by reducing symptoms Mechanism of action: Azathioprine is a chemical analogue of the purines adenine and guanine. Azathioprine works by interfering with the metabolism of purines which is required for the synthesis of DNA RNA and the proteins in T cells as a result it’s suppresses the immune response Pharmacological class: Purine antagonist; Antimetabolite Right right right time/frequency Azathioprine is the form of 75 MG tablets and the prescribed dose is 500 MG (2 tablets) to be taken orally once daily after meals. This dosing is appropriate as the dose in adults begins with 50mg a day and may be increased by 25 MG every 1 to 2 weeks to a target dose of 2– mg/kg/day. In the case of Mr. Polansky, who weighs 75 kg a daily dose of 150-225 mg is appropriate. In the case of Mr. Polansky, who weighs 75kg a daily dose of 550-225 mg is appropriate. The oral route avoids injections, and taking it with food helps reduce side effects Assessment Pre-medicate by evaluating liver, kidney and blood function. Monitor white blood cell and platelet count to watch for leukopenia and watch for signs of infection.

Right to refuse Mr. Polinsky has the right to refuse the medication if he does, we would educate him on the importance of taking as prescribed for his health goals. If he refuses, we could consider switching to another drug in the same class such as Esmolol. Mr. Polinsky has the right to refuse the medication if he does, we would educate him on the importance of taking as prescribed for his health goals. If he refuses, we could consider switching to another drug in the same class such as Esmolol,Acebutolol, Atenolol or Bisoprolol Education Direct the patient to take the medication at the same time every day and not stop suddenly this can result in a dangerous evaluation of blood pressure if a dose is missed. Take it as soon as remembered unless it is close to the next dose when the missed is omitted do not double the dosage. Right documentation It is essential to record properly if the medicine was administered or not, if it was refused by patient document it as well Right patient verification Check for the right patient prior to administering any medicine plays a crucial role in delivering safe and effective care Herbal/vitamin interaction Mr. Polinski is also taking echinacea (500 mg, three times daily) to prevent colds; this however, is not appropriate since the drug echinacea stimulates the immune system while azathioprine suppresses it. This antagonism can reduce treatment effectiveness and also increase the likelihood of a flare up. It is best that Mr. Polinsky avoid taking echinacea while on the azathioprine treatment
