COMPETENCY CHECKLIST
MALE CATHETERIZATION
Expected Outcome: Bladder is not palpable; minimum 30ml/hr in urine bag; patient verbalize relief from bladder
discomfort and minimal pain during procedure.
Rate the student as follows:
1 - Needs Practice 3 - Very Satisfactory
2 - Satisfactory 4 – Excellent

1. Verify physician’s order
2. Perform hand hygiene
3. Prepare needed equipment
4. Establish rapport
5. Verify the patient’s identity using two patient identifiers
6. Explain procedure to patient and/or significant others and
encourage patient’s interaction and questions.
7. Provide privacy by pulling the curtains around the bed and closing
the door
8. Raise the bed to a comfortable working height
9. Place underpad under the lumbosacral area of the patient
10. Place patient to supine or sitting position with thighs slightly apart
11. Drape with blanket
12. Put on clean gloves
13. Clean the tip of the penis first, moving the cotton balls in a
circular motion from the meatus using downward strokes toward the
pubic area
14. Remove gloves and perform hand hygiene again.
15. Prepare urine bag set up if separate urine collection system is to
be used. Secure to bed frame below level of bladder and do not
place bag on side rails
16. Open sterile catheter pack. Connect it to drainage tubing. Ensure
that urine bag is placed below level of the bladder
17. Put on sterile gloves
18. Lift the penis with non-dominant hand. Retract foreskin in an
uncircumcised patient. With dominant hand, pick up cotton balls
soaked with antiseptic solution. In a circular motion, clean the penis
from urethral meatus down to the base glans. Repeat this cleansing
motion two more times, using a new cotton ball each time
19. Lubricate the tip of the catheter
20. Lift penis with non-dominant hand with slightly upward tension
perpendicular to patient’s body
21. Ask patient to bear down gently as if to void, and slowly insert
catheter trough urethral meatus approximately 17-22.5 cm (7”-9”) or
until urine begins to flow. As soon as urine appears, advance
catheter another 2.5-5 cm (1”-2”) or until the bifurcation or “Y” level
of the port.
22. Lower penis and hold catheter securely in non-dominant hand.

  1. Use dominant hand to pick up prefilled syringe (10cc PNSS) and inflate balloon
  2. Pull gently on the catheter tubing after balloon is inflated until resistance is felt
  3. Secure catheter tubing to top of thigh or lower abdomen (with penis directed toward chest) with a tape. Allow some slack so leg movement does not create tension on catheter
  4. Hang the urine bag at the appropriated side of the bed
  5. Assist patient in comfortable position
  6. Properly dispose used materials according to infection control policies
  7. Remove gloves and perform hand hygiene
  8. Document procedure, amount and characteristics of urine as well as patient response to procedure TOTAL Formula: Add all items, divide by 120 and multiply by 100.
