RAAS
- โ†“ BP
โ†’ โ†“ BP flow to kidneys
โ†’ renin
- angiotensinogen โ†’ angiotensin 1
- lungs โ†’ ACE (angiotensin converting enzyme)
- angiotensin 1 โ†’ angiotensin 2
- vasoconstriction
- works on adrenal gland
โ†’ produces aldosterone
โ†’ works on kidney
โ†’ โ†‘ reabsorption of Naโ– 
โ†’ โ†‘ water reabsorption
โ†’ โ†‘ blood volume โ†’ increase BP
ACE Inhibitors (end in -pril)
- lisinopril, captopril, enalapril, benazepril
How they work:
- stop vasoconstriction
- stop increase blood volume
- decrease preload
- decrease afterload
- increase cardiac output
Side effects:
- hypotension
- cough โ†’ normally goes away in weeks
- increase potassium (hyperkalemia)
- rash โ†’ angioedema
Indications:
- hypertension
- CHF โ†’ life savers
- diabetic nephropathy
- post MI ventricular dysfunction

Notes:

  • does not work on African Americans
  • AA have high risk of angioedema
  • first pick for HTN meds ARBs (end in -sartan)
  • losartan, irbesartan, valsartan, candesartan How they work: same as ACE inhibitors Indications:
  • same as ACE inhibitors
  • migraines (candesartan works best) Side effects:
  • same as ACE inhibitors (less frequent & less intense)
  • insomnia, dizziness, pharyngitis
  • MI, renal failure, cancer Aldosterone Inhibitors
  • spironolactone Direct Renin Inhibitors
  • aliskiren Calcium Channel Blockers (CCBs)
  • diltiazem, nifedipine, amlodipine, verapamil Indications:
  • hypertension, prinzmetal angina, cardiac dysrhythmia Side effects:
  • hypotension, gingival hyperplasia, constipation, peripheral edema Alpha Blockers
  • terazosin, tamsulosin, prazosin, doxazosin Indications:
  • hypertension, BPH, PTSD Side effects:
  • hypotension (1st dose syncope), constipation, cystitis, dizziness, headache, sexual dysfunction Notes:
  • give @ bedtime
