NG tube insertion was taught to client and she understood the procedure as well as
why she was it was being placed. Prior to the insertion, hyperactive bowel sounds
present in all quadrants. Abdomen was lightly distended no tenderness upon
palpation. Client reported nausea, and bloating. During procedure a 14 French
Salem Nasogastric tube was inserted into right nare at 1630 and at 1645 X-ray
confirmed and verified placement of NG tube. NG tube placed on low intermittent
suctioning per physician order. Tube was secured at 47 cm. Gastric residual green
and output 250mL with a pH of 4. Client tolerated procedure and instructed keep
HOB at 30 degrees. Client is resting comfortably in bed with no complaint of pain.
