Anatomy

Anatomy of long bone

โ— Periosteum โ—‹ Fibrous layer: fibroblasts โ—‹ Cellular layer: osteoprogenitor cells โ†’ osteoblasts, osteoclasts โ—‹ Contains blood vessels, nerves, and lymphatic vessels that nourish compact bone

โ— Endosteum โ—‹ Inner membranous lining of compact bone that lines the medullary cavity

Blood supply

Nutrient / medullary artery system: (High pressure system)

โ— Enter the cortex then medullary canal through nutrient foramen (mid-shaft) โ— Branches into: ascending & descending โ†’ goes laterally to end of the bone โ†’ anastomosis with metaphyseal and epiphyseal vessels

Pathogenesis and Types of

Osteomyelitis

Common pathogens

Acute hematogenous osteomyelitis

Organisms reach the bone through the bloodstream.

โ— Adult (5%) โ—‹ Most common affected site = Vertebrae โ—‹ infection reaches the vertebral column through spinal venous plexus โ— โ— Infant and children (95%) โ—‹ Most common affected site = Long bone metaphysis

Contiguous-focus and posttraumatic

osteomyelitis

โ— Direct inoculation of bacteria โ—‹ Trauma, open fracture, surgical reduction and internal fixation of fractures, prosthetic devices โ—‹ โ— Septic arthritis โ—‹ Infection extend into the underlying bone โ†’ Erosions and osteomyelitis โ—‹ โ— Vascular compromise โ—‹ Diabetes mellitus are predisposed to osteomyelitis due to inadequate local tissue response โ—‹ Minor trauma to the feet โ†’ Multi-organism infection โ†’ Foot ulcers allow bacteria to reach the bone โ—‹ Patients may not experience any resulting pain, because of peripheral neuropathy โ†’ Delayed treatment

โ— Organisms reach the bone through the bloodstream from a septic focus elsewhere in the body โ— โ— Infection begins in the metaphysis of a long bone (productive medium for bacterial growth) โ— โ— It may spread to involve a large part of the bone. โ— โ— The organisms induce an acute inflammatory reaction but rigid structure of the bone not allow swelling. โ— โ— Pus is formed and soon finds its way to the surface of the bone where it forms a subperiosteal abscess

Sequel to acute osteomyelitis

โ— Abscess may burst into the soft tissues and form a sinus

โ— Usually the blood supply to a part of the bone will be cut off by septic thrombosis โ— โ— The ischaemic bone dies and eventually separates from the surrounding living bone as a sequestrum โ— โ— New bone laid down beneath the stripped-up periosteum, forming an investing layer known as the involucrum

Vertebral Osteomyelitis / Epidural Abscess

โ— Neurologic deficits present in 10-20% โ—‹ direct infectious involvement of neural elements โ—‹ compression from instability of the spine โ—‹ compression from an epidural abscess โ–  collection of pus or inflammatory granulation tissue between dura mater and surrounding adipose tissue

Implant-associated osteomyelitis

โ— Early onset (within 30 days of implant insertion) โ—‹ Staphylococcus aureus, Beta-hemolytic streptococci, Gram-negative bacilli โ—‹ Common cause: intraoperative contamination โ— โ— Late onset (>30 days of implant insertion) โ—‹ Staphylococcus aureus, Corynebacterium spp., P. acnes โ—‹ Frequently due to haematogenous infection

Symptoms โ— Dull pain at the involved site with or without movement โ— Refuse to bear weight (pediatrics) โ— Fever

Signs โ— Fever, tachycardia, hypotension โ‡’ sepsis โ— Erythematous, edematous limb โ— Tenderness over the affected bone โ— Increased local temperature โ— Restricted ROM โ— Draining sinus tract
