REMINDERS:

1. Any attachment should be properly labeled (pictures, forms,

illustrations etc)

2. Language and tone used should be formal

3. It should be neat, organized, precise and on time

4. Book old style is the font, 12” font size, double space.

5. When you submit your practicum time card and internship /

training certificate make sure that you will NOT SUBMIT the original

copy, ALL I NEED is the photocopy ok?

6. All papers given by me(your practicum coordinator) will be placed in

the practicum report.

7. Every meeting, we will have the checking of attendance and this will

be graded too.
