ELECTRONIC CONSENT

You are invited to participate in a research study, entitled “An Analysis on the Stressors Associated

with Chemical Engineering Students of Adamson University’’. This consent form asks you to allow the

researchers to gain your thoughts and opinions to enhance understanding of the topic. The study is

being conducted by Rhea Angeline Bauzon, Suzeth Candelaria, Hannah Ruth Rivera and Julaiza Salazar

of Adamson University, 900 San Marcelino Street, Manila, 100 Metro Manila.

The purpose of this research study is to determine the factors of stress and address coping

mechanisms in handling stressors. Participants in the study will contribute to a better understanding of

the impact of stressors to Adamson University Chemical Engineering students.

Participation in this study is completely voluntary. If you decide not to participate there will not be

any negative consequences. Please be aware that if you decide to participate, you may stop participating

at any time and you may decide not to answer any speciﬁc question.

The researchers will maintain the conﬁdentiality of the research records or data. If you do not want

to participate either simply stop participating or close the online survey window. But, if you agree to

participate, please ﬁll out the following:
