RATIONALE FOR IMPLEMENTING VALUE- BASED CARE IN A HEALTHCARE ORGANIZATION ◈ The reconnection of patients and clinicians  (^) Many patients do not feel they can speak to clinicians and be heard or express their feelings.  (^) By implementing value-based care within the healthcare organization promotes the relationship between physicians and patients significantly.  (^) It can improve the plan of care for patients because of the constant communication and patient involvement. (TDABC) Time-Driven Activity-Based Costing ◈ (^) Promotes cost accuracy for value-based healthcare systems and increases cost savings. ◈ (^) Helps with redesigns of services within healthcare to be the most efficient. ◈ (^) Easy access for patients due to efficient redesigns for patient care. TWO STRATEGIES TO IMPLEMENT VALUE-BASED

HEALTHCARE

Patient engagement and involvement in the processes based on their care ◈ (^) Key stakeholders: Patients (recipient) and Physicians (benefactor) ◈ (^) Methods of measurement: Patient surveys given shiftily by nursing staff to determine if the patients feel involved in their care and how often the physician came by

SUMMARY OF VALUE-BASED NURSING CARE READINESS

ASSESSMENT

◈Defined need

◆ The organization has established a necessity to incorporate value-based care within the system. The defined need is lack of staffing, not having a measurement method and lack of awareness of human/financial resources. Staffing within the organization is inadequate and the leading factor to all of the issues with patient care.

◈Readiness for change

◆ The organization seeks to change by accepting the need to change for the greater good of patients' health. ◆ Employees have many recommendations for change by suggesting nursing staff having safety to care for patients with the communication of physicians and patients on the same page.

◈Leadership and management support

◆ Leadership and management support is there and continues to grow every day within the organization. They reward nursing staff in recognition for involvement in committees that promote patient safety and quality care. The nurses feel that their voices are being heard, and having open communication with physicians and directors improve everyday patient care.

SUMMARY OF VALUE-BASED NURSING CARE READINESS ASSESSMENT ◈ (^) Time, resources, and personnel ◆ Time and resources are stretched thin at this time due to COVID-19, but in time many value-based care strategies can be implemented. ◆ Staffing is very low and not practical at this time. ◈ (^) Sustainment of the change ◆ My healthcare organization does not have a way to measure value- based care at this time currently. However, teamwork is rewarded with recognition and performance incentives. ◆ Over time these practices can occur because we have a common goal to incorporate more value-based care in our everyday workflow.

TWO ORGANIZATIONAL OPPORTUNITIES FOR IMPROVEMENT

  1. Staffing needs in all service lines ◈ (^) Lack of recruitment from Human Resources ◈ (^) Competitive health organization within five miles with higher pay
  2. The need for a nurse residency program ◈ (^) New grads do no receive an adequate orientation ◈ (^) Increased turnover to competitive hospitals

ANALYSIS OF ORGANIZATION’S LEVEL OF READINESS SCORE ◈ Value-Based Care (VBC) Readiness Score: 12  My organization has many of the tools to succeed in implementing value-based programs and strategies. Staffing and having a method of measurement is lacking. Overall, my organization is capable based on the score.
