1. A patient presents with a three week history of painless progressive jaundice with pale stool and
dark urine. On examination, he has a mass in the right upper quadrant. Describe the differential
diagnosis, investigation and management of this patient.
2. Write short notes on;
a) Ileofemoral thrombosis.
b) Complications of abdominal aortic surgery.
c) Medical management of intermittent claudication.
d) Indications for surgery in patients with peripheral vascular disease.
3. Write short notes on three of the following;
a) Symptoms, clinical signs and investigations of a patient with an ulnar nerve palsy.
b) Differential diagnosis and investigation of persistent backpain in a 55 year old lady.
c) Principles of managing a 23 year old man with an open fracture of the tibia.
d) Compartment syndrome.
4. A 22 year old man is brought to the Emergency Department resuscitation room following a road
traffic accident. He was the driver of a car which hit a wall at high speed. The paramedics tell
you that he has sustained chest injuries and he also has seat belt bruising. On examination he is
distressed and has a blood pressure of 100/60. Outline your initial management and
investigations.
5. Write short notes on four of the following;
a) Targeted therapy in breast cancer.
b) Magnetic resonance imaging.
c) Tumour markers.
d) Carcinoid Syndrome.
e) Role of radiotherapy in the management of common tumours.
6. Write notes on two of the following:
a) Describe the AJCC staging of melanoma.
b) Describe the management of a patient with possible necrotizing fasciitis.
c) Describe the symptoms and signs of a patient with carpal tunnel syndrome, including
possible operative approaches.

  1. A 65 year old male patient has undergone a large bowel resection for colonic carcinoma. On the first postoperative night, the patient is noted to be flushed, tachycardic and to have a blood pressure of 80/40 mmHg. In a short answer format please answer the following questions.

a) What is the definition of shock? b) What is the differential diagnosis for this patient? c) Describe the haemodynamic alterations seen in hypovolaemic shock, and contrast that with the alterations seen in septic shock. d) Describe how you would resuscitate this patient, and the initial steps in regard to diagnosis and definitive management.
