Please complete this form and return it, along with the Student Information Form (found in the Biology Major Manual), to the Biology Academic Office, Leidy Lab, room 102. When you are provisionally admitted to the major, you will be assigned to an advisor and these documents will be part of your major file. You will be discussing your course selections with your major advisor. The aim is to construct a combination that closely reflects your specific interests.
