NURSING SKILLS NOTES (SESSIONS 02–08)
SESSION 02 – ABDOMINAL & NUTRITION
- Abdominal assessment: Inspect → Auscultate → Percuss → Palpate
- Normal bowel sounds: 5–30/min
- Urine output: ≥30 mL/hr
- Aspiration: sit upright, chin tuck, stay upright after meals
SESSION 03 – SAFETY & NEURO
- Fall prevention: bed low, call bell, non-slip socks
- Restraints: last resort, monitor q2h
- Neuro: PERRLA, LOC, strength scale (0–5)
SESSION 04 – CARDIO & RESP
- Heart: S1 (lub), S2 (dub)
- Lungs: crackles = fluid, wheeze = narrowing
SESSION 05 – RESTRAINTS
- Require MD order, time-limited, frequent checks
SESSION 06 – WOUND CARE
- Red = healthy, yellow = slough, black = necrotic
- Moist wounds heal faster
- Sterile field rules apply
SESSION 07 – MEDICATIONS
- 6 rights
- Ear: up & back
- Eye: conjunctival sac
SESSION 08 – INJECTIONS
- SC: 45–90°
- IM: 90°
- Insulin: clear → cloudy
