Planning for our patients during times of transitions (for example: hospital to home, home to rehabilitation facility) involves collaboration with a number of healthcare professionals. Please address the following questions:

According to Chamberlain College of Nursing (2021), nurses have the huge responsibility of providing health teaching to avoid health complications and promote positive health behaviors. Nurses play an essential role in promoting successful transitions by developing a patient-centered transition plan and communicating any problem that might interfere with the results (Camicia & Lutz, 2016). In my workplace, when we receive new patients from the hospital, nurses have to educate the patient about the concept of dialysis and the importance of being compliant. Patients must be aware of the consequences of missing their treatments and of not controlling their water and sodium intake. When patients are transitioned from in-center hemodialysis to home hemodialysis, nurses must educate the patients and their families about the process, contraindication, and how to prevent any infection. Nurses also make sure patients go home with all their medications. One of the gaps I could identify is poor communication with the patient and family. Sometimes they leave with a lack of education or poor knowledge which can cause these patients to be readmitted and at risk of major problems. In addition, another gap could be that sometimes the providers do not follow up with the patients when they are transitioned from in-center to home hemodialysis, so it is difficult to evaluate if there was any improvement.
