Star Point 1: Discovery (Identify topic and practice issue)
Identify the topic and the nursing practice issue related to this topic. (This MUST involve a nursing
practice issue.)
Hand hygiene protocol to address the clinical practice issue of hospital-acquired infections (HAIs).
Briefly describe your rationale for your topic selection. Include the scope of the issue/problem.
The clinical practice problem of hospital-acquired infections (HAIs) is increasingly becoming a concern
in the healthcare sector. The hands of the healthcare professionals serve as vectors for transmitting
infections from one patient to the other. With the enforcement of hand hygiene protocol within healthcare
settings, the challenges of HAIs would be addressed and the patients will be cushioned from the problem.

Week 6 Assignment: EBPChange Process form

ACE Star Model of Knowledge Transformation

Follow Nurse Daniel as your process mentor in the weekly Illustration

section of the lesson.

Name:Marie Michel

Star Point 2: Summary (Evidence to support the need for a change)

Describe the practice problem in your own words and formulate your PICOT

question.

Most of the hospital-acquired infections that patients get are transmitted and caused

by poor hand hygiene of healthcare workers.

Population- Admitted patients

Intervention – Hand Hygiene

Protocol Comparison – Lack

of hand hygiene Outcome –

Reduced HAs

Time – Admission period.

List the systematic review chosen from the Cochrane Database of Systematic

Reviews from the Chamberlain library. Type the complete APA reference for

the systematic review selected.

Andriani, Y., & Nadjib, M. (2018). The Importance of Implementation of Hand

Hygiene Practice in Reducing Healthcare-associated Infections: A Systematic

Review. KnE Life Sciences, 4(9), 135-145.

List and briefly describe other sources used for data and information. List any

otheroptional scholarly source used as a supplement to the systematic review in

APA format.

Zhou, Y., Zhang, D., Chen, Y., Zhou, S., Pan, S., Huang, Y., & Ba-Thein, W.

(2014). Healthcare- associated infections and Shanghai clinicians: A multicenter

cross-sectional study. PloS one, 9(8), e105838.

In this source, the scholars of the main article used the literature therein to affirm

on the problemof HAIs in the hospital.

Briefly summarize the main findings (in your own words) from the systematic

review and the strength of the evidence.

From the article, Andriani and Nadjib (2018) assert that the use of hand hygiene

remains to be the most effective approach to addressing the continued crisis of

HAIs in healthcare settings. When the healthcare professionals ensure a high level

of hygiene in their hands, it eliminates chances of acquiring and transmitting

disease-causing organisms such as bacteria. Handling different patients exposes

healthcare professionals to a wide range of disease-causing organisms and

especially bacteria. If there is no hand hygiene within the healthcare facility,

healthcare provides transmit the disease to the patient.

The strength of the evidence is based on the fact that the scholars relied on 11 high-

quality research articles undertaken in different regions of the world such as

Europe, Asia, Middle East, China, and Thailand. This shows that the findings

derived from the studies in these areas were representative and independent of each

other.

Outline one or two evidence-based solutionsyou will consider for the trial

project.

Hand hygiene protocol among healthcare providers attending to the admitted

patients.

Star Point 3: Translation (Action Plan)

Identify care standards, practice guidelines, or protocols that may be in place to support your intervention planning (These may come from your organization or from the other sources listed in your Summary section in Star Point 2). The care standards include;- Washing of hands before and after attending to the patient Using the best sanitizing agent for washing of hands

Star Point 4: (Implementation)

Describe the process for gaining permission to plan and begin a trial. Is there a specific group, committee, or nurse leaders involved? The admitted patients are involved in the process and so is the team of the healthcare providers and especially nurses and physicians.

Describe the plan for educating the staff about the change process trial and

how they will be impacted or asked to participate.

The staff members will be trained and informed about the project especially on

washing their hands before and after attending to a patient. The healthcare

providers will achieve patient safety through their practice. Instead of being

vectors for transmitting infections from one patient to the other, they will be the

agent of patient safety across healthcare facilities.

Outline the implementation timeline for the change process (start time/end time,

what steps are to occur along the timeline).

The project will take place in 6 months to allow quantification of the available being

studied.

List the measurable outcomes based on the PICOT. How will these be

measured?

Measurable outcomes

1. Reduced incidents of HAIs

2. The length of stay at the hospital

What forms, if any, might be used for recording purposes during the pilot

change process. Describe. The forms used in the process include the one

monitoring the rate of incidents in HAIs. This will be used to determine whether

Star Point 5: (Evaluation)

How will you report the outcomes of the trial? The outcome will be reported in a written report and also presented in a PowerPoint Slide. The slide presentation will capture the pertinent issues of the project and some of the areas that need to be changed. What would be the next steps for the use of the change process information? Creation of clinical practice guidelines that guide healthcare providers to hand hygiene. The written CPGs will be given to healthcare providers to be referring to when undertaking their practice within the healthcare provider. study. The stakeholders will be reminded of the project that was introduced in the first meeting.
