Describe your search results in terms of the number of articles returned on original research and how this changed as you added search terms using your Boolean operators. Finally, explain strategies you might make to increase the rigor and effectiveness of a database search on your PICO(T) question. Be specific and provide examples. Main Post Introduction to Clinical Issue of Interest The clinical issue of interest that struck me was periprosthetic joint infection (PJI). PJI is a complex infectious process affecting surrounding tissues, bones, and implanted prostheses. It is usually caused by bacteria and fungi, mainly Staphylococcus aureus, with a high morbidity rate. The incidence of PJI following hip and knee arthroplasty is estimated at 1% to 2% in the One million hip and knee arthroplasties carried out yearly in the United States. However, this number is estimated to quadruple in the next 10- years (Ayoade et al., 2023). Strategic search with Boolean operator With the use of the Walden library and database, I decided to use the CINAHL Plus with text to research my topic, Periprosthetic joint infection alone in the Boolean/Phrase for a period of 10 years (2014 to 2024), 1,358 different articles were reported. Meanwhile, upon including some variables like etiology, 339 articles were brought forward. The time range can easily be adjusted to fit the timeframe required for research. These database engines are set up so that their usage can be efficient and effective, saving you time as a researcher. PCOT question is a format that helps formulate a search approach that produces the most relevant information and best evidence from existing literature (Melnyk & Fineout-Overholt, 2023). The PICO concept was introduced by Richardson et. al in 1995 to break down clinical questions into searchable words with mnemonics addressing different key questions. P - Patient or Problem: Who is the patient? What are the most important characteristics of a patient? What is the primary problem, disease, or co-existing condition? I – Intervention: What is the main intervention being considered? C – Comparison: What is the main comparison intervention? O - Outcome: What are the anticipated measures, improvements, or effects? T- indicates timeframe (Davies, 2011). In my clinical problem, P stands for the disease condition Periprosthetic joint infection, I- The nature of treatment involved, two-step revision, C- a comparison to those who are successful after surgery without PJI, the O- outcome will be Improved function after treatment, and T is the length of time to achieve healing.
