Burris Laboratory School Prom Guest Request Form A student requesting to bring a guest, who is not a Burris Laboratory School student, must have this form completed and approved by parents/guardians and an administrator of the guest’s high school. Guests must be in high school, or a recent high school graduate (20 years or younger) and must presenta current photo identification card at the door on the night of the dance. Guests 18 years, or older, must also complete the Criminal Background Check Authorization form (on back).
