Brenda โ€“ Patient received at start of shift in visit with family. Patient transitioned back to group
following visit without issue. Patient presented as calm and cooperative throughout this shift.
Patient ate food from home with family so did not eat their dinner. Patient was an active
participant in all milieu activities. Patient reported feeling disappointed and worried due to
uncertainty surrounding discharge plan. Patient was able to identify the benefits of being honest.
Patient was also able to appropriately identify the consequences of dishonesty. Patient had phone
calls with sisters which reportedly went well. Patient completed evening ADLs without issue.
Patient currently in room at time of this note.
Ella โ€“ Patient received at start of this shift playing monopoly with peers in dayroom. Patient
presented as calm, cooperative, and guarded this shift. Patient had visit with family which
reportedly went well. Patient ate 100% of their dinner. Patient was an active participant in all
milieu activities. Patient reported feeling happy due to seeing family today. Patient was able to
identify the benefits of communication with family and friends. Patient was praised for their
willingness to openly communicate feelings throughout this shift. Patient chose not to call
anyone during this shift. Patient currently watching movie with peers at time of this note.

presented as calm and cooperative this shift. Patient had visit with parents this shift which
reportedly went well. Patient initially refused dinner due to feeling tired but was able to eat about
50% later in the shift. Patient was an active participant in all milieu activities this shift. Patient
reported feeling content due to getting new clothes and having ice cream today. Patient identified
triggers for feeling mad/sad/anxious. Patient was also able to identify coping tools to use when
having those feelings. Patient reported feeling annoyed due to parents requesting further
treatment. Patient engaged well with peers this shift. Patient currently watching movie with peers
at time of this note.

Patient presented as calm and cooperative this shift. Patient had a visit with Dad this shift which
reportedly went โ€œokayโ€. Patient ate 100% of their dinner without issue. Patient was an active
participant in all milieu activities. Patient reported feeling guilty and worried about making their
Dad upset due to this hospitalization. Patient was able to discuss these feelings thoroughly with
group and reported feeling better after discussing. Patient was able to identify coping tools they
will be able to use outside of the hospital. Patient had phone call with brother which reportedly
went well. Patient currently watching movie with peers at time of this note.
