Postpartum Clinical Assignment

Instructions: You are to interview a mother, who gave birth within the last six
months. Write a brief summary of the mothers’ answers and create a plan of care
for this mother based on her response to the following interview questions:
1. When did you give birth to your child? (Date/Year)
2. Where did you have your baby? (home vs. hospital)
3. Did you or your baby have any complications after the delivery? If so please
list at least two that you remember.
4. Did you or your baby have any complications or problems at home after you
were discharged from the hospital? If so please list at least two that you
remember.
5. What would you say was the most difficult adjustment for you as a mother
postpartum? If any, describe two.
Your care plan should identify at least two nursing diagnoses for this mother , state
the long and short term goals for the proposed care, with a minimum of three
nursing interventions.
