1. Patient teaching concerning the use of anti-infectives is needed?
-Pt show signs of improvement in 3-4 days
-destruction of bacteria release overload of toxins in body
-may feel worse for the first 1-2 days (chills, fever, malaise)
-Finish whole prescription even if feeling betting (avoid development of resistant
bacteria)
-Stopping early = body has not yet effectively killed out the bacteria making you
sick
????? -Patient finished all of the prescription; antibiotic did not kill his infection but
just slowed its growth.
– “This is okay because your body will help kill the bacteria too”
-photosensitivity. Will predispose the pt to sunburn
-Female pt: antibiotics will interfere with oral contraceptive
-nausea, vomiting and stomach pain
-take on an empty stomach to ensure complete absorption unless they cannot
tolerate. Take with water increase fluid intake to prevent
Teaching – Instruct patients not to take with milk or antacids, which will bind and delay or
prevent absorption (particularly tetracycline).
-INFECTION: secondary infection: normal flora is disturbed by large dose of
antibiotic therapy.
-caution female pt to be alert for vaginal yeast infections. Instruct to eat
yogurt
2. Gentamicin (Garamycin) priority nursing
action? Aminoglycoside -myCIN – potent antibiotic
**Draw – Peak and Trough Level
Initial trough 0.3-10 just prior to second dose
Late trough 10.3-1.0. Obtain on day 7
Peak 5-12. 45-60 mins after the end of the infusion
-Assessment
❑Record vital signs and urine output.
❑Assess lab results of renal and liver function tests.
❑Obtain a medical history related to renal and hearing disorders.
-Adverse Effects
❑Ototoxicity
-hearing loss
❑Renal toxicity
❑Headache
❑Paresthesia
❑Fever
❑Superinfections
❑Vertigo
❑Skin rash
❑Dizziness
-Nursing diagnosis
