Discuss how the application of nursing-quality indicators could assist the nurse in this case If the nurses in this scenario were to use the nursing-quality indicators such as restraint prevalence and patient satisfaction it would allow them to better care for this patient. The restraints that were used on Mr. J did not seem necessary and therefore could be avoided. Using the nursing-quality indicator the nurse could decide if Mr. J’s pressure ulcer was developed within the facility or if he was admitted with it. If the ulcer was acquired during his hospital stay, then restraints could have contributed to that, since he would not be able to move himself around in the bed like a patient normally would. Also, Mr. J received meat on his meal tray that went against his religion, and that will greatly decrease patient/family satisfaction. The nurses were not vigilant on apologizing or even making the family aware of the food mistake, or why Mr. J had restraints on in the first place. B. Analyze how hospital data could advance quality patient care With data readily available for the nurses to view, we can better understand how often patients are getting pressure ulcers, being put in restraints, or overall having a bad experience.
We as nurses can use this data to come up with solutions to help prevent or lower the prevalence of these incidences and be more vigilant to prevent them. Ways to use the data to prevent a pressure ulcer would be to assess for patients that are high risk, such as bed ridden patients, and make sure they are turned or repositioned at least every 2 hours. Patients who are mobile need to get up and walk around the room or move to the chair about every 2 hours to help avoid a pressure ulcer. We could also use the data to understand how often restraints are used and if they are always necessary for those patients. If a patient is not in harm’s way, restraints should not be used, the nurses should just assess the patient more frequently. Another way to analyze data for patient-satisfaction would be to assess the scores for the hospital or unit specific scores and try to address needs that are decreasing the scores. If patients are complaining that call lights are not being responded to in a timely manner, that needs to be fixed. Although nurses are busy, we need to strive to give the best care to our patients daily, including responding call lights promptly. C. Analyze the specific system resources, referrals, or colleagues that you could use to resolve an ethical issue in this scenario As a nursing shift supervisor, I would help to avoid the ethical dilemmas that occurred in this scenario in many ways. First, I would make sure that my nurses were well trained on ethics and what to do if mistakes were made, such as the wrong meat, and not telling the family. Since the patient’s daughter had spoken to the physician which in turn contacted the hospital supervisor, I would take advantage of that and use the administrative staff to correct the issue. The patient’s daughter is clearly upset about the situation, and that needs to be addressed. I would have every person or department who was involved, including dietary, to come and personally apologize to
