This is a large amount of money, particularly for someone who is retired and may live on a fixed income. It would be difficult to cut down on some of the exams, they help narrow down the diagnosis. I may have to ask my patient what their budget is and then make a decision as to what is most vital to order. Also, I would see if there are any resources available for him like blood work at a community center. I may consider removing the 12-lead, maybe the perfusion scan, CT scan, and stress test. The criteria for diagnosing COPD is debatable according to Petrie et al., 2021, but universally as providers we should be focusing on patient symptoms and spirometry diagnostics like post-bronchodilator spirometry tests.
Data indicate that clients with chronic lung disease and low income have increased utilization of acute care services (Melillo, 2021). What questions should the primary provider ask when providing education to clients with these diagnoses to ensure they have the resources they need to follow up with appropriate care? I can start by asking my patient if they need physical support, emotional support, or social support. Living with COPD can drastically reduce a person’s wellbeing and limit their quality of life. Ask your patient if there is a particular area they need assistance with. If they struggle for example with social support, they want to keep up with family and friends. Offer pulmonary rehabilitation, this can help the patient learn the skills they need to manage their COPD and stay active (American Lung Association, 2024).
