- An incident that happened with a patient in the facility resulted in the patient sustaining injury due to staff error. While my preceptor was not
the one to commit this error, she took the time to really listen to the patients' concerns, frustrations, and needs. She allowed the patient to then collaborate with her and find the best course of action. She then escalated the concerns up the chain of command until she was able to bring someone in who could further help the patient on the customer service end of things. The reason I thought this was a shining representation of professionalism is because she gave the patient autonomy and validated their experience. I have seen many times, providers brushing things off that didn’t necessarily “pertain to them”. She not only listened, but ensured the person above her would also listen. She showed transparency and trustworthiness in my opinion, two very important professional values.
- I experienced my preceptor showing scholarly service to her peer's mid-way through the course. There was a new NP that came on board to round for a GI doctor that my preceptor had previously worked for. The NP was new to the field and new to GI and was very nervous and unsure. She felt like she didn’t have the support of her doctor and was “thrown to the wolves”. Each day after my preceptor and the new NP meeting, my preceptor would sit her down for a few minutes each day and go over the patients and teach and offer insight to the new NP. My preceptor had no benefit to doing this as she no longer worked for this doctor and hadn’t for over a year. I hope to run
act of service to healthcare, passing on knowledge because it’s the right thing to do. - During my time at Encompass, I was exposed to my preceptor being an active agent in change as well as applying her vast knowledge to her care there. My preceptor has been a NP for over a decade and has worked in a variety of specialties as well as primary care. During my time there, we had a patient that was in need of a C.diff Sample that ended up being delayed due to lack of communication. My preceptor respectfully and kindly presented a solution to the problem to ensure that the patient’s labs were appropriately obtained, and there was a clear line of communication and standardization. She brought this knowledge from a prior facility that she worked out which I appreciated. After she shared her thoughts on improving the flow of labs (urine/stool in particular), we did not experience further hang up’s revolving around miscommunication. In my opinion, she acted as a change agent in this particular scenario. Without the sample, our patient remained in preemptive isolation for too long, meaning she could not share a room and another patient would then be denied a
bed. Further, she ended up being positive for C.diff, requiring intervention that was delayed due to delay in specimen collection.
- We had a patient that had a very curious presentation, and I used the education on scholarly sources to further research his presentation. I ended up finding multiple articles on Doege- Potter syndrome which was aligning with what we were seeing in this case. I took the information to the Pulmonologist who was interested and passed it on to the hemo/onc doctor. While they felt that it was not the cause of our patient's presentation, I was commended both on my ability to research and the interesting information I brought to the table. During our time together, there was a policy in place that the only doctor that could make referals to other specialists was the rehab doctor. This caused a delay in care as well as “missed opportunities”. The way my preceptor explained her discomfort was that the rehab
to allow internal medicine to make appropriate referrals due to their different perspective of the patient. She felt that she was able to get the patient specialized care sooner and prevent further complications if she would be allowed to make her own referrals during her intake portion of the patients stay. Ultimately, they allowed this to happen, and Serena stated she felt her patients were getting quicker medical care, allowing them to have a better rehabbing experience.
